Farmington, WI – Fatal Two-Vehicle Crash at CTY TK H & Forest View Rd

Farmington, WI (April 10, 2026) – One person died and another was hurt after a two-vehicle crash Wednesday afternoon, April 8, in the Town of Farmington. The collision took place at the intersection of County Trunk Highway H and Forest View Road.

Emergency units responded around 4:20 p.m. after multiple 911 calls reported a serious accident. Deputies arrived and found two SUVs with heavy damage at the intersection.

The driver of a gray SUV was unresponsive when crews reached the scene. First responders attempted life-saving efforts, but the driver did not survive.

Authorities said the driver of a blue SUV remained conscious and alert after the crash. That individual suffered injuries and received medical attention, though officials have not shared the extent of those injuries.

Early findings show the blue SUV was traveling west on CTY TK H as it approached Forest View Road. The gray SUV traveled eastbound and attempted to turn left onto Forest View Road, where it entered the path of the oncoming vehicle and was struck on the passenger side.
